What is a Caregiver Authorization?

Start with the task the caregiver needs to perform, then check the required form and authority. A school pickup list may not be enough for a medical decision, and a travel consent letter may not cover either.

What a caregiver authorization covers

  • Who the child is — full legal name and date of birth for each child, matching what the doctor's office has on file.
  • Who the caregiver is, with an address and a phone number that will actually be answered.
  • A start and an end date. An open-ended authorization signed years ago invites the question of whether it is still current, and a stale one is easy for an office to refuse.
  • Collection and day-to-day care — the specific tasks requested, checked against the school or provider's requirements and any custody restrictions.
  • Medical care — confirm what authority local law permits, who can grant it, and which document the provider needs. Do not assume a generic clause authorizes admission, anesthesia or every treatment.
  • How to reach you, plus a second contact on a different schedule.
  • Health details — allergies and what to do about each, current medications, the insurance policy, the pediatrician.

A caregiver authorization is not guardianship

A private permission letter does not by itself establish court-appointed guardianship. Check the law where the child is being cared for and any custody order before relying on it.

For a state-specific example, California court guidance on caregiver affidavits explains that a qualifying caregiver can use an affidavit for school enrollment and school-related medical care; relatives may have broader medical-consent authority. The affidavit does not require the parent's signature and does not grant legal custody. Those rules should not be assumed to apply to a generic letter or another state.

The mistake that makes one useless

People write the letter for the caregiver and never give a copy to the people who have to act on it. Ask the school and healthcare provider in advance which documents they accept and how to submit them securely. Give the caregiver an accessible copy in the required format. A document nobody can produce at the counter has not done its job.

Keep parent and backup contact details current, and tell the caregiver when each contact is likely to be reachable. Ask the provider how it handles consent questions when a parent cannot be reached.

They overlap and answer different questions. A caregiver authorization answers "may this adult look after and consent for this child?" and its audience is a school, a daycare, or a clinic. A minor travel consent letter answers "may this child leave with this adult?" and its audience is an airline check-in desk or a border officer. Check travel-document requirements and medical-consent authority separately for the itinerary. Neither letter guarantees acceptance or replaces required custody documents.
