Once you know how something turned out you cannot un-know it, and the version of your reasoning that comes back to you has quietly been edited to fit the ending. That is why a decade of big decisions can leave you feeling like a good decider with no evidence at all. This free sheet is the fix in one page: write down the call, the options you turned down, your reasoning, how sure you are, and one prediction specific enough to be marked right or wrong — then give it a date and come back. What it does

  • One page per decision: the call, the options you turned down, your reasoning in your own words, and how reversible it is.
  • A confidence number from 50 to 98, and one prediction with a claim, a way to test it, and a date — so it can actually be marked right or wrong later.
  • A review strip to fill in on the date you set, closing the loop against what you actually wrote rather than what you now remember writing. Put that date in your calendar as you fill the sheet in — paper cannot remind you.
  • Ships with a worked example filled in — an illustrative decision, its reasoning, its prediction, and the review — then a blank sheet to print as many times as you like.
  • Print it and write on it: US Letter, and it prints cleanly on A4 with "fit to page". Nothing to install, nothing to sign up for.
